Instructions (please read)
- IMPORTANT: After filling out the form, you must PRINT the form BEFORE proceeding to PayPal to submit payment.
- Print three copies of this form. One copy should go with each of the books that go to the three judges in your entry’s category.
- You must fill out all required fields to print, pay the submission fee and submit the form.
- At the bottom of the form, you will be required to pay the $25 submission fee. If you wish to pay by credit card, but DO NOT have a PayPal account, click “PayPal Checkout” and then select “Pay with Debit or Credit Card”. If you wish to pay using your PayPal account, click “PayPal Checkout” and login to your account.
- Awards Entry Form deadline for submission is January 5, 2020.
Note: We need author contact information to communicate with winner: editors only will disqualify the book.
